To enter the Competition, you will need to complete an Entry Form (in PDF format). Please complete a separate form for each brand.
(Please read General Rules first, then download the Entry Handbook 2010*.)

Download Entry Form in PDF format *. (If having problem downloading, simply right-click over PDF format link choose "Save target as" option.)
After you have fully completed the Entry Form on your computer or in pen, you may:

  • fax it to (562) 435-7118, or
  • send it to:

    (DO NOT ship wines to this address)


    If you choose to fax your Entry Form, you must (postal) mail your (check) payment immediately afterwards, referencing the name of your winery/company used on the Entry Form and the date the form was submitted.
